Health history forms are required prior to your initial visit. When you book online you will be sent an email with confirmation of your appointment and a link to fill in your health history form through the secure booking site. Please be as descriptive as you can as it will help during your assessment.
Paper forms are available when you arrive for your appointment for those wanting to use that method.
